Dual use, which is common, is at least as dangerous and likely more dangerous than smoking conventional cigarettes or using e-cigarettes alone. Further, not all ENDS are the same and the risks to health may differ from one product to another, and from user to user. E-cigarettes are still fairly new, and more research is needed over a longer period of time to know what the long-term effects may be.
When the human body breaks down a foreign substance, one can typically find chemical by-products in hair or urine that provide clues about how it has interacted with cells. This is true for nicotine, but in the case of propylene glycol, no one has established what the relevant by-product is or how to best detect it. Boston, MA – Diacetyl, a flavoring chemical linked to cases of severe respiratory disease, was found in more than 75% of flavored electronic cigarettes and refill liquids tested by researchers at Harvard T.H. Chan School of Public Health. Two other potentially harmful related compounds were also found in many of the tested flavors, which included varieties with potential appeal to young people such as Cotton Candy, Fruit Squirts, and Cupcake. In conclusion, the prevalence of susceptibility among adolescents differs by product type. Liking school and academically oriented educational aspirations appear as consistent protective factors, while the use of other tobacco or nicotine products and positive attitudes towards product use in one’s age group appear as risk factors.
Before this study — which Truth Initiative funded — there hasn’t been any data available on how to help teens quit vaping, according to the researchers. According to the study authors, prior research has already linked the nicotine in e-cigarette vapor to lung cancers in mice. "The amount of smoke the mice were exposed to was similar to a person who had vaped for about three to six years," they noted. "This study presents clear evidence showing that vaping in addition to smoking can increase your risk for lung cancer," said Bittoni, who works at the university's Arthur G. James Cancer Hospital and Richard J. Solove Research Institute. One of the ingredients in tobacco is the mood-altering drug nicotine.
According to the 2023 National Youth Tobacco Survey, more than 2 million teens reported vaping nicotine, and there was an increase in current overall tobacco product use among middle school students (4.5% to 6.6%) and multiple tobacco product use (1.5% to 2.5%). They are not technically categorized as smokeless tobacco, so the Food and Drug Administration (FDA) does not regulate them as strictly as smoked tobacco products or combustible tobacco. Without long-term data, we cannot be certain if or how reduced exposure will prevent harm to one’s health. In Canada, most of the provinces require smokers to be 19 years of age to purchase cigarettes (except for Quebec and the prairie provinces, where the age is 18). However, the minimum age only concerns the purchase of tobacco, not use. Alberta, however, does have a law which prohibits the possession or use of tobacco products by all persons under 18, punishable by a $100 fine.
Enrollees in the program receive interactive daily text messages tailored to their sign-up date or their target quit date if they set one. Program users who set a quit date (which they can change) receive messages for at least one week prior to the quit date and for at least two months following the quit date. Originally, it was thought that vaping might be helpful for people trying to quit smoking. Some e-cigarette labels have claimed that the product had no nicotine when, in fact, it was in the vapor. For this reason, it’s important to use only trusted brands if you vape. E-cigarettes are still relatively new, so their long-term effects are unknown.
Of note, the abstinence rate found in the nicotine-replacement group was lower than what is usually expected with this therapy. Nevertheless, the incidence of throat and mouth irritation was higher in the e-cigarette group than in the nicotine-replacement group (65.3% vs. 51.2%, respectively). Also, the participant adherence to the treatment after 1-year abstinence was significantly higher in the e-cigarette group (80%) than in nicotine-replacement products group (9%) [100]. These data overall indicate that the harmful effects of nicotine should not be underestimated. Despite the established regulations, some inaccuracies in nicotine content labelling remain in different brands of e-liquids.
Most factory-made cigarettes are equipped with a filter; those who roll their own can buy them separately. Filters can reduce some substances from smoke but do not make cigarettes any safer to smoke. A cigarette is distinguished from a cigar by its usually smaller size, use of processed leaf, different smoking method, and paper wrapping, which is typically white. Not everyone is aware that there is nicotine in most e-cigarettes in varying amounts. Some pods of e-cigarettes contain as much nicotine as one pack of cigarettes (Truth Initiative, 2019).

Of note, another study indicated that although RANTES/CCL5 and CCR1 mRNA were upregulated in flavour/nicotine-containing e-cigarette users, vaping flavour and nicotine-less e-cigarettes did not significantly dysregulate cytokine and inflammasome activation [43].

However, a major concern about vaping is its attraction for young people. And, while it’s safe when taken orally as a supplement or used on the skin, it’s likely an irritant when inhaled. It’s been found in the lungs of people with severe, vaping-related damage. Some of these additives have health risks, such as diacetyl, which has a buttery taste. Diacetyl has been found to cause a severe lung disease similar to bronchiolitis. Use of electronic cigarettes (e-cigarettes) impairs indoor air quality and increases FeNO levels of e-cigarette consumers.
The most important points to know are that the long-term health effects of e-cigarettes are still unknown, and all tobacco products, including e-cigarettes, can pose health risks to the user. The tobacco industry has co-opted the term harm reduction to further its own interests of reduced tobacco product regulation and expand its user base. E-cigarette use among adults has been more stable, with adult use remaining relatively low since around 2012. Recent data, however, showed a slight uptick in adult e-cigarette use from 4.5% in 2021 to 6% in 2022. While switching completely from cigarettes to e-cigarettes can be less harmful than smoking, many adult smokers use e-cigarettes and cigarettes concurrently — increasing overall exposure to nicotine. The cytotoxic and pro-inflammatory effects of different e-cigarette flavouring chemicals were also tested on two human monocytic cell lines—mono mac 6 (MM6) and U937 [86]. Also, all the tested flavours produced significant levels of ROS in a cell-free ROS production assay. Finally, diacetyl, pentanedione, O-vanillin, maltol, coumarin, and CAD induced significant IL-8 secretion from MM6 and U937 monocytes [86]. It should be borne in mind, however, that the concentrations assayed were in the supra-physiological range and it is likely that, once inhaled, these concentrations are not reached in the airway space.

Despite this trend all current evidence finds that e-cigarettes carry a fraction of the risk of smoking. The 966 respondents who had never vaped more often believed that e-cigarette waste was dangerous to throw in trash compared to the 1,083 respondents who had vaped at least once (81.4% vs. 71%). Beginning August 1, 2019, Minnesota law prohibits the use of these products indoors where cigarette use is prohibited, including bars and restaurants. A 2021 study found that daily e-cigarette usage among tobacco smokers can increase the likelihood of quitting smoking eightfold. Researchers assessed data from the 2014–2019 Population Assessment of Tobacco and Health (PATH) study, focusing on smokers who were not planning to quit smoking at the start of the period. However, a 2019 study into the long-term health effects of vaping found that people using e-cigarettes had a higher risk of respiratory disease than people who never smoked. The authors first created a comprehensive database of tobacco product flavor prohibition and restriction laws across the United States, including both state and local statutes. Electronic nicotine dispensing systems (ENDS), commonly known as electronic cigarettes or e-cigarettes, have been popularly considered a less harmful alternative to conventional cigarette smoking since they first appeared on the market more than a decade ago. Both the veeba electronic cigarette devices and the different e-liquids are easily available in shops or online stores. The long-term health effects of electronic cigarettes (e-cigarettes) are not fully understood, but the science indicates they are not a safe alternative to smoking. Most are noncombustible which includes a battery, a heating element, and a liquid compartment, usually containing addictive nicotine, that is added to the e-liquid or included in the device. The heating element aerosolizes the liquid for the inhalation of the liquid nicotine or other contents.
Vaping has exploded into a national crisis, and tobacco companies are helping to fuel it by targeting kids with flavors such as gummy bear and cotton candy. E-cigarette companies have promoted unsubstantiated health claims about their products as healthier than traditional cigarettes, when, in fact, e-cigarettes are uniquely dangerous for kids due to nicotine’s impact on their developing brains. It's also hard to know exactly what chemicals are in an e-cigarette because most products do not list all of the harmful or potentially harmful substances contained in them. This was a large increase from 2019, when only 11.3% of people had ever used e‑cigarettes, and 2.5% were currently using them (Figure 1). The proportion of people who used e‑cigarettes daily in 2022–2023 was 3.5%, rising from 1.1% in 2019. In smokers who smoke tobacco cigarettes, blood flow increased modestly after cigarette inhalation and then decreased with subsequent stress. However, with those who vaped, blood flow decreased after inhalation at rest and also after handgrip stress. Finally, lung health effects are not expected to be seen for 20 to 30 years.
Long periods of dual use of cigarettes and e-cigarettes can result in harms to health similar to, or in addition to, the harms from exclusive use of cigarettes. This is called secondhand vaping, and there isn't a lot of published research yet on how inhaling this aerosol affects the body, especially among adolescents. Separate studies have shown that newer e-cigarettes may deliver higher doses of nicotine more efficiently, which could explain the recent findings. The investigators acknowledged that more frequent use of e-cigarettes may also reflect greater determination to quit smoking combustible cigarettes. They noted that their study did not evaluate the risks of youth e-cigarette use. E-cigarettes are the most commonly used commercial tobacco product among youth,1 so it is critical that public health officials and the general public understand the potential risks of using them.
In this article, we discuss the risks of vaping versus smoking and consider the long-term effects and risks of both. In light of these results, policymakers might want to consider other approaches to protect public health where tobacco is concerned, the authors wrote in the study. Of the increase in cigarette sales, 71% were of non-menthol cigarettes, suggesting that restrictions on menthol cigarettes would not counteract this effect. The primary outcome of interest was volume sales of conventional cigarettes and e-cigarettes per capita, during each four-week period, both overall and by type of flavor. Cigarette volumes are measured in packs, while e-cigarettes are standardized to 0.7-milliliter units, given past industry claims that one 0.7-milliliter vape pod is equivalent to 20 conventional cigarettes, or one pack.
But e-cigarettes haven't proved to be safer or more effective than nicotine-replacement medications in helping people stop smoking.
