"From a public health perspective, we have always been concerned about dual-use of both traditional and e-cig products," said study lead author Marisa Bittoni, an oncology researcher at Ohio State University in Columbus. The U.S. Food and Drug Administration and the European Union are grappling with these issues as they decide how to regulate the products. Unfettered access could leave people vulnerable to unknown health hazards, but there is also the chance that greater restrictions might hurt folks who are trying to forgo conventional—and more dangerous—tobacco products.
According to Blaha, there are three reasons e-cigarettes may be particularly enticing to young people. Second, e-cigarettes have a lower per-use cost than traditional cigarettes. State and local authorities can restrict the sale of flavored tobacco products, including e-cigarettes.

Not only is there evidence of mislabelling of nicotine content among refills labelled as nicotine-free, but there also seems to be a history of poor labelling accuracy in nicotine-containing e-liquids [37, 38]. "Since there is no safe tobacco product, eventual abstinence from all tobacco products is the end goal," concluded senior study author Andrew Hyland, PhD, Chair of Health Behavior at the Roswell Park Comprehensive Cancer Center. Meanwhile, people who were smoking e-cigarettes non-daily had a 3.1% rate of quitting smoking and a 10.2% rate of cutting down to non-daily tobacco smoking. However, the CDC states that there is insufficient evidence to suggest vaping can help people quit smoking. A second brand of e-cigarettes -- marketed as Nixotine, Nixodine, Nixamide and Nic-Safe – contained a nicotine analog called nicotinamide, also at levels lower than the labels indicated, and combined with undisclosed amounts of 6-methyl nicotine. The nicotine analogs were included in flavored e-cigarettes, which prior research has indicated are preferred by youths and those who vape for the first time.
At present, reference products for e-cigarette testing are not available. E-liquids are an important part of any vaping system and their composition, together with the characteristics of the device, may have an impact on nicotine delivery21. They mainly constitute a mix of propylene glycol (PG), glycerol (vegetable glycerine or VG) and nicotine. E-liquids may include flavouring compounds and usually come in different nicotine strengths or concentrations. Meanwhile, the government has so far issued 14,795 challans under Section 6 of The Cigarettes and Other Tobacco Products Act, 2003, which prohibits the sale of cigarettes and other tobacco products to and by persons below 18 years. However, many young users report they began using e-cigarettes because of the flavors and, according to CDC, do not realize they are inhaling nicotine. Other compounds that have been detected in aerosols include acetamide, a potential human carcinogen [5], and some aldehydes [69], although their levels were minimal. Interestingly, the existence of harmful concentrations of diethylene glycol, a known cytotoxic agent, in e-liquid aerosols is contentious with some studies detecting its presence [4, 68, 70,71,72], and others finding low subtoxic concentrations [73, 74]. In this regard, either it was detected at concentrations that did not exceed the authorised limit [73], or it was absent from the aerosols produced [4, 71, 72]. Only one study revealed its presence at high concentration in a very low number of samples [5]. Nevertheless, its presence above 1 mg/g is not allowed by the FDA [73]. Figure 1 lists the main compounds detected in aerosols derived from humectant heating and their potential damaging effects.

The difference in opinion worldwide may be due to different restrictions imposed. Tetrahydrocannabinol (THC)-containing e-cigarette or vaping products was linked to most EVALI cases. Emergency department visits related to e-cigarette, or vaping, products have sharply declined in recent years, but the CDC continues to monitor reports. In 2022, e-cigarettes remained the most common tobacco product used by high school (16.5%) and middle school (4.5%) students in the last 30 days. And more than a quarter (27.6%) of current youth e-cigarette users say they use an e-cigarette product every day.
The American Academy of Pediatrics and the American Lung Association are glad to have the rules. But, Sward adds, "We certainly thought [the FDA] should have gone farther -- ending the sale of flavored products." Some come in candy and fruity flavors that appeal to kids and teens. Sward points out that according to the FDA, there's no evidence any e-cigarette is safe and effective at helping smokers quit. She suggests talking to your doctor about medications and other strategies that are proven stop-smoking tools. There is an urgent, overdue need for standardized processes for the disposal of e-cigarette devices, refills and e-liquids. Waste management and hazardous waste disposal plants are not currently equipped to handle e-cigarette waste and federal regulations still have not caught up to the need for guidance on disposal of these types of small electronics containing toxic waste.
Given their relatively recent introduction, there has been little time for a scientific body of evidence to develop on the health effects of e-cigarettes. Notably, the reverse was not true—students who said they smoked cigarettes were no more likely to report use of e-cigarettes when asked approximately 6 months later. Electronic cigarettes, or e-cigarettes, are tobacco products that have been sold in the U.S. for about a decade. They include e-pens, e-pipes, e-hookah, and e-cigars, known collectively as ENDS—electronic nicotine delivery systems. They're also sometimes called JUULs, "vapes" and "vape pens." E-cigarettes are the most commonly used tobacco products among kids—and it's become an epidemic. While much remains to be determined about the lasting health consequences of e-cigarettes, there’s evolving evidence about the health risks of e-cigarettes on the lungs—including irreversible lung damage and lung disease.
Participants took one vape from an e-cigarette, with at least 12 mg/mL of nicotine, or inhaled a conventional cigarette, every 20 s for 10 min. Blood samples were collected 1, 2, 4, 6, 8, 10, 12 and 15 min after the first puff, and nicotine serum levels were measured by liquid chromatography-mass spectrometry (LC–MS). The results revealed higher serum levels of nicotine in the conventional CS group than in the e-cigarette group (25.9 ± 16.7 ng/mL vs. 11.5 ± 9.8 ng/mL). However, e-cigarettes containing 20 mg/mL of nicotine are more equivalent to normal cigarettes, based on the delivery of approximately 1 mg of nicotine every 5 min [40].
Results of toxicological analyses suggest that e-cigarettes can be safer than conventional cigarettes, although harmful effects from short-term e-cigarette use have been described. Worryingly, the potential long-term effects of e-cigarette consumption have been scarcely investigated. In this review, we take stock of the main findings in this field and their consequences for human health including coronavirus disease 2019 (COVID-19). Heart failure is a condition affecting more than 6 million U.S. adults in which the heart becomes too stiff or too weak to pump blood as effectively as it should.
Dual use, which is common, is at least as dangerous and likely more dangerous than smoking conventional cigarettes or using e-cigarettes alone. Further, not all ENDS are the same and the risks to health may differ from one product to another, and from user to user. E-cigarettes are still fairly new, and more research is needed over a longer period of time to know what the long-term effects may be.
One of the first studies in humans involved the analysis of 9 volunteers that consumed e-cigarettes, with or without nicotine, in a ventilated room for 2 h [8]. Pollutants in indoor air, exhaled nitric oxide (NO) and urinary metabolite profiles were analysed. The results of this acute experiment revealed that e-cigarettes are not emission-free, and ultrafine particles formed from propylene glycol (PG) could be detected in the lungs. In line with these observations, dysregulation of lung homeostasis has been documented in non-smokers subjected to acute inhalation of e-cigarette aerosols [10]. Globally, there is an ongoing debate about whether e-cigarettes are a threat or an opportunity to public health.
This ban excludes disposable e-cigarette devices, which are sold in many appealing flavors and are relatively inexpensive. And are now the most popular type of e-cigarette used by adolescents. Nicotine is highly addictive and may lead to nicotine cravings and development of tolerance (the need to use more often in order to have the same effect). Nicotine addiction can negatively affect relationships and impair performance at school, at work, or in other activities. Researchers have also identified vitamin E acetate, a chemical added to some THC-containing vaping products, as the main—but possibly not the only—cause of the illness.
This data highlights a small part of a larger issue, despite the ban on e-cigarettes and vapes in India under the Prohibition of Electronic Cigarettes (Production, Manufacture, Import, Export, Transport, Sale, Distribution, Storage, and Advertisement) Act, 2019. The Annual Review of Physiology article summarizes what is known—and highlights where gaps in knowledge exist—about e-cigarettes and harm to the lungs. The review discusses the range of effects found in rigorous studies, including shorter-term acute effects and longer-lasting chronic impacts. As substantiated in the review, a convincing body of evidence exists that confirms e-cigarettes cause lung inflammation and injury, as well as negative health effects in multiple organs. Crotty Alexander says e-cigarettes come with far more dangers than even she expected.
A Foundation-led study presented at the 2024 Society for Research on Nicotine and Tobacco annual meeting found that monthly e-cigarette sales in California declined by 41% after statewide restrictions on the sale of flavored tobacco products took effect. However, the report also noted that the tobacco industry continues to market products to offset these declines, including non-menthol cigarettes that contain other cooling chemicals. Continuing to monitor flavored tobacco product sales is key to assessing policies like these and informing further policy development and implementation. Toxicity is not the only factor in considering risk to an individual or a population from exposure to ENDS emissions.

Consequently, stricter regulation and a higher quality control in the e-liquid industry are required. The mislabelling of nicotine content in e-liquids has been previously addressed [8, 34]. Of note, several studies have detected nicotine in those e-liquids labelled as nicotine-free [5, 35, 36]. E-cigarettes, also known as e-cigs, vapes, vape pens, and electronic nicotine delivery systems (ENDS), are experiencing rapid growth in popularity, especially among teens and young adults. They come in a variety of forms, sometimes looking like USB flash drives, pens, and other items that make them hard for parents and teachers to spot. Other electronic devices that heat tobacco instead of liquid nicotine, such as IQOS, are not considered e-cigarettes. Teens cannot participate in such studies, but "we all agree that e-cigs are not a good thing for youth and nonsmokers," Dr. Baldassarri says. The Food and Drug Administration on Friday authorized the first menthol-flavored electronic cigarettes for adult smokers, acknowledging that vaping flavors can reduce the harms of traditional tobacco smoking.
This includes the biennial WHO Report on the Global Tobacco Epidemic, which tracks the status of the tobacco epidemic and interventions to combat it and other relevant resources. Because of the lack of long-term data and the fact that patients have died from EVALI, the prognosis for those affected remains uncertain. Researchers are working hard to learn as much as possible about the illness, its causes, and the odds for making a full recovery. "We recommend that patients see a pulmonologist within one to two weeks after being discharged to undergo testing of lung function and pulse oximetry [level of oxygen in the blood]," she says.
Originally, it was thought that vaping might be helpful for people trying to quit smoking. Some e-cigarette labels have claimed that the product had no nicotine when, in fact, it was in the vapor. For this reason, it’s important to use only trusted brands if you vape. E-cigarettes are still relatively new, so their long-term effects are unknown.
In general, most of the cytokines detected in BALF were significantly increased in WT mice exposed to PG with nicotine compared to PG alone or air control [42]. Some of these effects were found to be through nicotine activation of NF-κB signalling albeit in females but not in males. In addition, PG with nicotine caused increased macrophage and CD4+/CD8+ T-lymphocytes cell counts in BALF compared to air control, but these effects were ameliorated when animals were sub-chronically exposed to PG alone [42]. Researchers know more about the long term effects of electronic cigarettes-term effects of smoking than those of vaping. However, vaping products contain a great deal fewer harmful chemicals than cigarettes and can be an effective tool in quitting smoking altogether.
The results showed that people who used e-cigarettes at any point were 19% more likely to develop heart failure compared with people who had never used e-cigarettes. In calculating this difference, researchers accounted for a variety of demographic and socioeconomic factors, other heart disease risk factors and participants’ past and current use of other substances, including alcohol and tobacco products. The researchers also found no evidence that participants’ age, sex or smoking status modified the relationship between e-cigarettes and heart failure. While teen smoking has fallen by 80% over the past 20 years, a new generation is now at risk of becoming addicted to nicotine — and possibly at risk for other serious health problems — through e-cigarettes. More than 2.5 million U.S. middle and high school students are now using e-cigarettes, with nearly 85% of them using flavored products.

Smoking is the leading cause of lung cancer, and people who smoke are 20 times more likely than nonsmokers to be diagnosed with lung cancer. Last year, 9.4% of individuals ages 12 years and above reported vaping nicotine within the previous month, up from 8.3% in 2022. "This research has provided greater understanding in the court systems nationwide as to the nature of drug usage, abuse, and overdose cases in which electronic cigarettes were used to deliver an illicit drug," the researchers concluded. THC, synthetic cannabinoids, and dextromethorphan, a drug found in some cough medicines, "were identified in e-liquids purportedly containing only CBD to be consumed for ‘health benefits,’" the researchers said. Results from the National Drug Strategy Household Survey do not provide evidence for or against the use of e-cigarettes as a smoking cessation tool, or as a gateway to tobacco smoking. The age groups most likely to smoke tobacco in 2019 and 2022–2023 were the least likely to use e-cigarettes, and the age groups most likely to use e-cigarettes had low tobacco smoking rates in 2019 and 2022–2023. In 2022–2023, 1 in 5 (19.8%) people aged 14 and over in Australia reported having used e‑cigarettes at least once in their lifetime, with 7% currently using e‑cigarettes at least daily, weekly, monthly, or less often than monthly.
